Proctology has seen major advances in the field recently where there is paramount shift from open procedures to minimally invasive procedures and LASER surgery.

Dr Maruthesh Gowda has vast experince in Open, minimally invasive and LASER Proctology.

Haemorrhoids treatment has moved away from painful open haemorrhoidectomy. Haemorrhoids are now treated by Banding & Sclerotherapy in the initial stages. Larger/Later stages haemorrhoids are treated by Haemorrhoidal Artery Ligation & MucopexyOR LASER (Hemorrhoidal Laser Procedure (HeLP). Both these procedures can be done as day care and cause minimal pain post procedure. People should be able to resume routiene work few days later.

Fistula surgery has also progressed recently with move away from open surgey to minimally invasive procedures in the form of VAAFT (Vedio Assisted Anal Fistula treatment) and LASER fistula treatment (Closure of fistula-in-ano with laser—FiLaC). Both procedures can be done as day care and with minimal pain. There is significant reduction in the post operative morbidity. People should be able to resume work in matter of days

Pilonidal sinus surgery has moved away significantly from Excision and flaps to minimally invasive EPSiT (Endoscopic Pilonidal Sinus Treatment)& LASER Ablation of Pilonidal track. Both procedures can be done as day care with minimal scarring, pain and early return to routien.

Anal Fissure patients who do not respond to medical treatment now can have LASER Lateral Internal Sphinctorotomy Or Closed Lateral Internal Sphinctorotomy (CLIS). With wound of about 0.5cms people are hardly able to see the scar from surgery.

In general Protology procedures have moved away from open procedures to minimally invasive procedures and LASER procedures greatly benifitting the patients with less pain, less scar and significanlty reduced mortality. Patients are able to return to normal with in few days.

Laser treatment, condition by condition

Piles (Haemorrhoids)

Piles are painless and cause bleeding, itching and sometimes jelly-like secretions, but they very rarely cause pain.

3rd and 4th degree haemorrhoids need surgery. It's better if it's done when its 3rd degree as LASER surgery is an option and gives better results.

LASER has now revolutionised treatment haemorrhoids. This is newer way of dealing with haemorrhoids. The biggest advantage is it combines haemorrhoidal artery ligation and LASER so one gets better result compared to other procedures. Pain associated is very minimal. This can be done as day care procedure. Currently we are using 1470 LASER which has no lateral spread beyond the haemorrhoids hence no complications.

Anal Fissure

Anal fissure is the most common cause for painful defecation (Opening bowels). Most times it is also associated with bleeding but not all the time. It is a small cut in the anal canal upto skin.

When treated appropriately in the initial stages, fissure heals upto 75% with medicines. Longer the duration less likely it is to heal with creams and might need simple surgery to relax the sphincters so that fissure can heal.

LASER has now revolutionised treatment of Anal Fissure. One can have LASER Lateral Sphincterotomy with minimal incision, Less pain and quicker recovery. Wound from the surgery is hardly visible

Anal Fistula

It is an abnormal connection between your anal canal (back passage) to the surrounding skin.

The only way to cure a fistula is by surgery. LASER in fistula surgery is the newer way of dealing troublesome condition. It has several advantages over other procedures like

  • Sphincter is not injured so there is no compromise on continence. This is the biggest advantage compared to all the other procedures making it safe and widely acceptable.
  • Wound will be minimal leading to faster recovery and early return to work.
  • Pain is minimal allowing patients to mobilise quickly
  • No damage to surrounding tissue.

There are different methods of using LASER in Fistula surgery and it can be combined with other treatment modalities like Vedio Assisted Anal Fistula Treatment (VAAFT) leading to higher success rates while causing minimal pain and disruption to one's work life balance.

Pilonidal Sinus or Pilonidal Abscess

Pilonidal is a condition in which hair grows/collected under the skin in natal cleft (Space between two buttocks).

LASER has now revolutionised treatment of pilonidal sinus. All the treatment options discussed above involve big scar, pain and intensive wound management. In addition they alter ones work/life balance significantly for about 6 -8 weeks. LASER Surgery overcomes all the disadvantages of the traditional surgery. Advantages are

  • Scar is minimal seen only at pits
  • Pain is minimal compared to other procedures
  • Wound management is minimal
  • One can return to work quicker

Procedure is usually combined with Endoscopic Pilonidal Sinus Treatment (EPSiT) leading to higher success rate. These advantages make LASER treatment for Pilonidal sinus widely acceptable.
